While controlling the victim's actions is nice, I like the idea that the victim does it to themselves…

Maybe control a bunch of bees to surround them until they overheat? Like the Japanese honeybee?

Pull a Mortal Kombat if you have a pretty sharp katana! Spin it like a helicopter blade and push them through it like one of those meat slicers you find at the deli! Delicious!

Wait, is that true? I'm not sure how well hemoglobin coagulates when cooked, but that would be quite interesting seeing as many "secret ingredients" that villains use involve human body parts.
Speaking of cooking, if the telekinetic could move atoms quickly enough to generate heat, you could literally boil their blood! Even better, you could change the state of matter and either have things like their blood or eyes solidify or make their teeth melt out of their face!

You could use the telekinesis to manipulate the blood in their bodies so that it stops moving or you could force them to cough up all of their blood. Your choice.

Make their digestive system crawl out of their mouth and wrap around their head until it suffocates them.

Pop their eyeballs out of their head and use them to plug up their windpipe.

Blow up each vertebrae in the spinal column. Implosion of the spinal vertebrae works too.

Murder them by making a small telekinetic bubble in their brain, and expanding it until their head starts to slough off.

Make a small air bubble between their toes, which would make it seem like a heart attack.

Explode their eyes from the inside out, and repeat this with everything else.

Make their bones twist around in their body, and disconnect.

I like the "swallowing the eyes" bit, it reminds me of that one scene from The Dark Crystal where those puppets rolled their eyes on the ground, yelled "Snake Eyes!", picked them up and threw them so that they caught them in their mouth and replaced them back into their sockets.

  • In Star Wars and the Grishaverse, there's a type of ability that can slow down bodily functions telekinetically, so you could have someone slowly lower their enemy's heartbeat until they keel over, or stop their blood from flowing to certain places, like the brain.
  • Any form of blood manipulation is nasty but effective
  • Rupture their internal organs, or alternately, slowly destroy them over time by making small tears in them and letting the acids wreck their insides
  • Directing a lodged thing like a bullet from a former injury to another location (e.g. if the bullet is in their leg, move it through their guts until they are destroyed

if you want a pretty sad way of dying have them make it look like a suicide and if not just make their heart implode on itself. If you want another sadder one, have someone they care about dearly stab them to death and then kill them off. You could also toy with them for a long time like making them hurt themselves or hurt the ones they love before actually killing them.

man this is good! inner demon, time to play!
you know what's worse than killing someone with telekinesis? using it to not let them die. tear someone's limbs off and teeth out, but stop them from bleeding, make them beg for death, make them think that killing them is mercy.

or pull them to pieces, but keep everything intact. the veins and organs stay connected and plugged in to the right spots, but take the skin off and bones out, spread the body out.
splinter the bones and use the fragments to stab your foe to death

(Taking these from Mortal Kombat 10 and 11):
–Lift a guy up, rip off their limbs, and then slam them back into the torso like a crushed soda can!
–Spin a sword like a helicopter blade and push them into it slowly like a blender!
–Push down on their head so hard that their spine telescopes and they have to live the rest of their short life with no neck. They'll look like a Goombah!
