Mine's not really super scary, but the Cursed Doll. Basically she was a china doll (terrifying I know) with a spirit inhabiting her, and on the mental plane she could take the shape of your worst fear. In the book I wrote, she became blood, a clown and someone's abusive father.

My mermaids from Neverland (Purgatory):
They have masses of tangled seaweed for hair, webbed ears and hands, dark dark green skin and scales that vary in color. They have large doe-like eyes that give off a sense of innocence, and then two rows of razorsharp teeth. They consume anything and everything they can get their hands on, but they love fairies and souls the most, especially souls with a lot of energy and emotion.

Personally I'm most terrified by two creatures I've created:
The Woolly Maw which is like a enormous white bear (about 33 feet tall) with sharp claws and a long tail which is all muscle. It has an gigantic mouth with huge teeth and what appears to be a beak as a nose, and small slit black eyes. It lives in the snowy north and can detect even the smallest movement from beneath the snow from a mile away, and has great sense of smell. Once sensing a creature nearby, it will burst from its den and charge at it.
The last is my interpretation of the Luska, a massive "blob" the size of a two story house covered with tentacles, and has a huge mouth with four rows of teeth, the lower jaw pointed at the end which usually wraps above its upper jaw, and extends open as large as its own body.

I came up with a monster awhile ago, Radeghast. This monster, supposedly from another place other than Earth, can somehow travel through long distances using emotion. Well, other peoples' emotions. It will "grasp" their emotions if they're strong enough to reach far enough, and Radeghast will travel to this person. Radeghast found Earth because scientists wanted to find life outside of their world, and their curiosity was grasped by Radeghast. These scientists find it, and they also find that this Radeghast can manipulate light waves around it, so it always has darkness surrounding it. In this case, no one can actually describe what Radeghast looks like, but if they were to describe it they would see an eight-foot monster covered in shadows with arms growing and shrinking from its whole body, and they would also see two eerie white specks that indicate eyes. Radeghast can also copy humans like what parrots do, and it mysteriously always seems to find the scariest quotes for the worst times, evidently to lure or scare. Radeghast also has a taste not for human flesh but their bones, and it also loves to toy with emotions. By "toying," I mean leaving bodies of loved ones in the least expected places, or, say, when you hear your dead brother's voice echoing in your bedroom.

On the collection of her last dream she wears a black robe, as a symbol of mourning. She is always in the form of a loved one, technically she slips her will or mind into yours/your loved one and drinks in the dreams through physical or emotional connection, if strong enough. On occasion she will kidnap a soul in her true form. You know that falling sensation that you get sometimes? That's her failing at taking you away. Name suggestions?

Okay buckle up fam. I'd like to contribute 'A, the literal embodiment of chaos/mental illness/death/decay/etc. It's from one of my stories. Basically, it can take whatever form it wants, but it's very bad at mimicking real things and creatures, so usually its form looks really….weird. Example: It may try to become a human, but it forgets to add skin. Or maybe it tries to become a cat, but it has beetles instead of eyes. Faces are the hardest. Usually it just settles on a shapeless grey mass that constantly shifts and warps where the face should be. Its voice is always changing too, based on the voices it hears. It's very curious of humans, but doesn't know how to treat them. Sometimes it will accidentally kill one while trying to make friends, and not realize for a few centuries because it can talk to ghosts. Spending too much time around it can drive you insane.
